Instructions for your testing services, such as drug testing or health screenings, are provided in a separate email. This email will include instructions, attachments, and a link stating, “Show the barcode on mobile device” containing documentation that must be scanned at the collection site to begin your screening. Please take the barcode and a valid picture I.D. with you to the address below to complete your screening. If you have been given additional documents related to your drug and health screening, please take those as well.

Valid Picture I.D. (not expired) acceptable examples: State DL, State ID, Passport, or School ID. If you do not have a valid picture I.D., please contact your hiring company as soon as possible.

We highly recommend that you call the collection site in advance to confirm hours of operation and availability. Please note that your expiration date and time is NOT an appointment. You must report to the collection site before your registration expires.

Parents with minors please read:

We apologize for any inconvenience; however, children will not be allowed into the testing area with you. If you have small children accompanying you to the testing facility, please arrange to have another adult with you to supervise them in the waiting room.
